Summary of FAA initiatives arising out of the Fiji investigation
In December 5 2006, on the day that Fiji was overtaken by a military coup, a Robinson 44 helicopter crashed off the coast of Nadi. The Fijian government appointed Andrew McGregor as Investigator in Charge (IIC) under the international ICAO convention. The National Transportation Safety Board of USA assisted with the investigation under Appendix 13 of ICAO. The NTSB has issued five recommendations to both the FAA and the manufacturer in order to address deficiencies which the investigation revealed.
